How Our Care Management Team Supports Patients Beyond Medical Care

At Finger Lakes Community Health (FLCH), care is more than medical visits and prescriptions—it’s about people, their lives, and their futures.

Our Care Management team works alongside patients to address the many non-medical factors that affect health. Whether it’s making sure prescriptions are filled, offering support for education and employment, or connecting individuals to financial assistance, housing, transportation, food, and insurance—we’re here to help every step of the way.

Staying Ahead with Preventive Measures
Preventing crises before they start is one of our top priorities. For example, running out of critical medications can pose serious health risks. That’s why we encourage patients to refill prescriptions early (please allow up to 72 hours for processing at FLCH), set reminders to call the pharmacy or provider about five days before medications run out, and explore delivery options when transportation is limited.

We also send appointment reminders via text. Through the Patient Portal (Healow App), patients can access test results, schedule visits, message providers, and more—all from their phones.

Navigating Financial Hardship
Applying for financial aid often feels overwhelming. The process includes paperwork, deadlines, and complex requirements—but no one has to go it alone. Our Care Management team helps by assisting with forms and documentation, coordinating appointments and evaluations, and ensuring everything is submitted on time. We’re here to simplify the process and reduce the stress that often comes with seeking help.

Hard to get a ride to your appointments?
Transportation should never be a reason to miss essential services like medical appointments, housing assistance, or accessing food. While Medicaid transportation is limited to medical visits, our team can help patients access local transportation resources, coordinate with community partners and provide rides in select cases through FLCH.

Food and Housing Support

A stable home and consistent access to nutritious food are the foundations of good health. Our team can help you apply for food assistance, offer information about local food pantries, suggest smart shopping tips, and give guidance on stretching budgets while choosing healthy foods.
For those in need of housing, we assist with applications for county housing programs, researching community-based housing support, and collaborating with local landlords to find safe, affordable homes. When people have housing, they can better focus on their health.

Health Insurance Enrollment and Support
Losing insurance coverage—especially after pandemic-related changes—can be devastating. We help patients avoid gaps in coverage by connecting them to our FLCH Financial Advocates, who will assist with applications for Medicaid and other insurance programs or will explore other income-based plans for those who don’t qualify for Medicaid. FLCH also offers interest-free payment plans if they cannot pay in full at the time of service. If a patient is experiencing a qualifying hardship, we offer up to a 100% courtesy discount.

A Holistic Approach to Health
Our Care Management team understands that good health is influenced by more than just physical conditions. Challenges such as low income, lack of education, childcare needs, and mental health issues all affect a person’s well-being. That’s why we also help patients with:
• Job training and resume help.
• Employment and interview preparation.
• Daycare assistance applications.
• Advice on balancing work and benefits.
