How to get started
To gain access to our secure server to the Patient Portal and become web-enabled, simply sign up by providing us with a personal (non-work) e-mail address. If you’d like to do it by phone or email, you will also need to provide us with your identifying information such as your name, date of birth, and emergency contact. Get Patient Portal access now by calling your health center or email us.
Once you become web-enabled, you will be able to securely log in with your username and password, and gain access to your personal health record and other helpful features from any computer or smartphone with an internet connection. The Patient Portal Login is here.
