Finger Lakes Community Health’s award-winning outreach program is designed specifically to meet the unique needs of migrant farmworker patients. The goal of the outreach activities is to provide preventive healthcare and a means to access healthcare if needed.

Community Health Workers are Certified Medical Interpreters, and representative of the cultures they serve, travel the Finger Lakes region searching out isolated pockets of farmworkers, passing out information, and offering a variety of patient services, such as community resources, patient education, referrals, transportation, home visits, interpretation, and more.

These services ensure that patients are able to obtain the healthcare they need, including preventive, diagnostic, and treatment services.
